Please use this form to safely and promptly provide information regarding an incident involving a Victoria Minor Hockey Association Player, Team, Staff Member, Volunteer or Event or even someone not attached to VMHA (a neighbouring association/tournament etc)

This reporting system is designed to be a safe reporting method for incidents that our members have experienced, witnessed, initiated or caused, that may have a negative impact on others. We understand that there has been a culture of silence within the Hockey World and are endeavoring to remove that silence and provide all those affected with a safe place to report such incidents to be reviewed by our Vice President and our Board of Directors if necessary or appropriate.
Victoria Minor Hockey Association takes the conduct of our players, staff, volunteers and board members seriously. We wish to foster a culture of inclusivity, safety and transparency.
In this form, you have the option to request a response or not. You may provide identifying information if you choose, but it is not required. You also have the option to remain anonymous.
Please note, that whether you choose to remain anonymous or not, your identity will be kept confidential.
The responses collected in this form are received by our Vice President and will be kept confidential outside of the Board of Directors, and only shared with the Board if it is necessary and appropriate. If you have reason to request that it be shared only with certain members or only the Vice President, please indicate the same. If you require a person other that the Vice President to make your report to, please reach out to any other board member for assistance.
Please also note that, as citizens in the province of BC, we are acutely aware of our duty to report situations to the authorities where a child or youth has been or is likely to be abused or neglected, or if someone is in danger of harming themselves or others.
